The following forms must be completed and returned to our Admissions Associate, Ms. Emma Entao, for your child to be considered for admission to The Beacon School.
- Completed application form
- Photocopy of parents’ passports and visas
- Photocopy of child’s birth certificate
- Photocopy of previous school records
- Photocopy of special needs reports (if any)
- Child’s medical report (from doctor)
- Emergency Authorization Form
- Documented proof of guardianship (if child is not living with parents)
- 2 pieces of 2 x 2 color photos of the prospective student
In addition, an Application Fee of Php 2500 is payable upon submission of all documents. Payment of this fee is required prior to a child being enrolled.
